Admission Process for Skilled Nursing and Memory Care

At The Wesley Community, we understand the difficulty and challenges that you are faced with when learning about obtaining admittance into a long term health care facility. Our purpose is to make this process as simple and pain free as can be. Below is a brief outline on how to get started getting your loved one admitted into our facility.

1. Pre-Admission

The Pre-Admission stage starts with a phone call or email inquiry of our services, and getting set up with a tour of our facilities and meeting with our admissions team. We will go over the admission forms with you, and review your financial and insurance information. We will also begin the development of a care plan with a health assessment by our nursing staff.

2. Day of Admission

On the day of admission, we will introduce you to our primary care team, and give you an orientation of the facility and room. You will receive assistance settling into your room and becoming familiar with the room amenities and the emergency call system. Our key staff will introduce themselves to you as well: RN Unit Coordinator, RN Charge, Social Worker, Dietician, and Activities Associate.

3. First Week

The first week we will perform a comprehensive and detailed physical and mental health evaluation, and do a medication review. Discussions with the family and resident will happen in order to finalize the care plan and make adjustments based on health assessment findings. A family meeting will then occur to discuss the care plan and expectations from our facility.

4. Weeks 2-4

The following weeks will consist of consistent and regular health checks and adjustments to medication and treatments. If therapy is required, initiation to those services will start as well. We will then focus of getting the resident to be a part of our social activities and group events, and to build relationships with other residents.

5. End of First 30 Days

At the end of the first 30 days a comprehensive review will be had with the resident and family, and the care plan will be evaluated and adjusted. Daily routines for care and activities will be solidified, and we ensure that the resident is comfortable and satisfied with the services provided. The Wesley Community discuss with the resident and families about the long term plan, and schedule regular reviews and meetings to ensure we are on track with those long term plans.

6. Ongoing Support

After the first 30 days, we continue to give continuous access to social workers and support services, and have availability for counseling and mental health support. We also encourage family to be involved with their loved ones care, and provide updates and open communication about their loved one.
